CMS Published CY2027 Home Health Final Rule; Retaining -3% Home Health Rate Reduction and Majority of New Enrollment Changes.
In its Calendar Year 2027 Home Health Prospective Payment System Rate Update published in the Federal Register on November 4, 2026, the Centers for Medicare and Medicaid Services (CMS) said it will finalize a 3.0% temporary reduction to home health base rates in CY2027. Combined with a 2.1% market basket increase for CY2027, the agency estimates that Medicare payments to home health care agencies in the coming year will increase in the aggregate by 2.4%, or $415 million, compared to CY 2026. While we acknowledge a positive payment update is better than previous years of cuts, LeadingAge strongly disagrees with CMS’ assessment that the observed behavior changes in CYs 2020 through 2022 are solely attributable to the implementation of the PDGM and the 30-day unit of payment; as noted in our August 31, 2026 comments, both CMS and the Medicare Payment Advisory Commission (MedPAC) have noted the inability to distinguish changes in provider behavior vs. consumer behavior especially during COVID-19 and in light of fraudulent actors. We will continue to work with Congress to ensure adequate payment to home health agencies. While we are still in the process of reviewing and analyzing the final rule, most of CMS' Medicare enrollment provisions were finalized as proposed, with a few notable exceptions, including CMS' decision not to finalize its proposal to deny enrollment based on an expanded definition of individuals and entities related to the provider with Medicare debt or payment suspensions. HOPE Changes: What’s New, What Providers Need to Watch.
While these advocacy-driven updates to the Hospice Outcomes and Patient Evaluation (HOPE) tool do not change the language of the actual questions in the HOPE tool the revisions are clarifications on coding instructions and guidance on how and when to ask and record information. LeadingAge has summarized all the key changes for members here.
